A common example is a middle-aged adult with deep true periodontal pockets, gingival recession, tooth mobility, furcation involvement, and radiographic evidence of alveolar bone loss. Another example includes localized attachment loss due to aggressive toothbrushing, presenting as cervical recession with exposed CEJ and true pocket formation. Key identifying features include irreversible attachment loss, apical migration of the JE, and true periodontal pocket formation. Pseudopockets are commonly seen in younger patients with plaque-induced gingivitis. For example, a teenager may present with swollen, edematous gingiva, probing depths of 6–10 mm, a hidden CEJ, and no radiographic bone loss. Similarly, patients with orthodontic brackets may develop localized gingival swelling around brackets, resulting in 5–7 mm probing depths without attachment loss. == Goals of Pharmacometabolomics == Pharmacometabolomics is thought to provide information that complements that gained from other omics, namely genomics, transcriptomics, and proteomics. Looking at the characteristics of an individual down through these different levels of detail, there is an increasingly more accurate prediction of a person's ability to respond to a pharmaceutical compound. The genome, made up of 25 000 genes, can indicate possible errors in drug metabolism; the transcriptome, made up of 85,000 transcripts, can provide information about which genes important in metabolism are being actively transcribed; and the proteome, >10,000,000 members, depicts which proteins are active in the body to carry out these functions. Pharmacometabolomics complements the omics with direct measurement of the products of all of these reactions, but with perhaps a relatively smaller number of members: that was initially projected to be approximately 2200 metabolites, but could be a larger number when gut derived metabolites and xenobiotics are added to the list. Overall, the goal of pharmacometabolomics is to more closely predict or assess the response of an individual to a pharmaceutical compound, permitting continued treatment with the right drug or dosage depending on the variations in their metabolism and ability to respond to treatment. == Treatment == In most of the reported cases, the treatment options were very similar. Plasmapheresis alone or in combination with steroids, sometimes also with thymectomy and azathioprine, have been the most frequently used therapeutic approach in treating Morvan's Syndrome. However, this does not always work, as failed response to steroids and to subsequently added plasmapheresis have been reported. Intravenous immunoglobulin was effective in one case. In one case, the dramatic response to high-dose oral prednisolone together with pulse methylprednisolone with almost complete disappearance of the symptoms within a short period should induce consideration of corticosteroids. In another case, the subject was treated with haloperidol (6 mg/day) with some improvement in the psychomotor agitation and hallucinations, but even high doses of carbamazepine given to the subject failed to improve the spontaneous muscle activity. Plasma Exchange (PE) was initiated, and after the third such session, the itching, sweating, mental disturbances, and complex nocturnal behavior improved and these symptoms completely disappeared after the sixth session, with improvement in insomnia and reduced muscle twitching. However, one month after the sixth PE session, there was a progressive worsening of insomnia and diurnal drowsiness, which promptly disappeared after another two PE sessions.
